To become a member of the Salt Lake Homeopathy Private Healthcare Membership Association, we ask that you read and fill out a contract, which is available as an Adobe PDF document. Our membership fee is only $10, and you'll have access to our consulting and products once you become a member.

If you have a scanner, you can also scan the contract and e-mail it as an attachment to saltlakehomeopathy@gmail.com

You do need to send the $10 application fee and you will also receive a $10 credit towards your purchase.
